REFSafE: A RAG-Enabled Framework for Predictive Risk Analysis and Automated Safety Report Generation in Mission-Critical Environments

Operational safety in mission-critical environments requires AI systems that are accurate, interpretable, and resistant to hallucination. We present an agentic Retrieval-Augmented Generation (RAG) framework, REFSafe, for grounded hazard analysis and automated safety report generation. The system integrates Large Language Models (LLMs) with structured operational data, historical incident repositories, policy documents, and external authoritative sources. Through iterative agentic reasoning, the framework retrieves, verifies, and synthesizes evidence prior to generation, enforcing citation-backed outputs with explicit source attribution (documents, links, and prior events) to ensure traceability and trust. To mitigate hallucinations and unsupported claims, all risk assessments and forecasts are constrained to retrieved evidence, with confidence signals derived from retrieval relevance and source consistency. A transparent pipeline enables subject matter experts (SMEs) to validate predictions, and provide structured feedback, forming a continuous performance calibration loop. Preliminary deployment demonstrates improved reliability in hazard detection and safety/vulnerability report generation. This work advances trustworthy, evidence-grounded AI for predictive safety intelligence in mission-critical operations.

Automating Bug Report Classification with Few Shot Learning

Orthogonal defect classification (ODC) is a method used to categorize software defects, providing valuable insights into the development process. This study focuses on automating the classification of software bug reports into different ODC defect types using few shot learning, a machine learning approach that requires minimal labeled data. Previous research has manually classified bug reports or used traditional machine learning algorithms like linear support vector machine, achieving limited success. Our approach uses few shot learning to improve classification accuracy and efficiency. The results show a harmonic mean of recall and precision (i.e., the F1 score) of around 0.6 which is a performance improvement over previous methods. The results highlight the potential benefit of few shot learning techniques and their application in enhancing the safety and reliability of nuclear digital instrumentation and control (DI&C) systems. Future work will explore incorporating advanced techniques to supplement the model's training data and achieve better results.

Universal Workflow Language and Software Enable Geometric Learning and FAIR Scientific Protocol Reporting

Written language and conventional data structures for representing scientific procedures suffer from low process detail, often fail to accurately represent protocols, and lack universality. New strategies for the handling of experimental data are needed to provide viable process information for both humans and machines. In this work, we present the universal workflow language (UWL) and interface (UWLi). UWL is a findable, accessible, interoperable, and reusable (FAIR)-compatible, graph-based data architecture that can capture arbitrary scientific procedures through workflow representation, and UWLi is an accompanying software package for building, manipulating, and interpreting UWL entries. The UWL format was found to be highly effective in identifying deficiencies in the reported process details of high-impact, peer-reviewed scientific journals, and in simulated scenarios, the graph format was shown to be more effective than conventional methods in predictively modeling the outcome of diverse scientific protocols. Implementation of UWL could enable more accurate scientific communication and more impactful process datasets.

Deep active learning for classifying cancer pathology reports

Abstract Background Automated text classification has many important applications in the clinical setting; however, obtaining labelled data for training machine learning and deep learning models is often difficult and expensive. Active learning techniques may mitigate this challenge by reducing the amount of labelled data required to effectively train a model. In this study, we analyze the effectiveness of 11 active learning algorithms on classifying subsite and histology from cancer pathology reports using a Convolutional Neural Network as the text classification model. Results We compare the performance of each active learning strategy using two differently sized datasets and two different classification tasks. Our results show that on all tasks and dataset sizes, all active learning strategies except diversity-sampling strategies outperformed random sampling, i.e., no active learning. On our large dataset (15K initial labelled samples, adding 15K additional labelled samples each iteration of active learning), there was no clear winner between the different active learning strategies. On our small dataset (1K initial labelled samples, adding 1K additional labelled samples each iteration of active learning), marginal and ratio uncertainty sampling performed better than all other active learning techniques. We found that compared to random sampling, active learning strongly helps performance on rare classes by focusing on underrepresented classes. Conclusions Active learning can save annotation cost by helping human annotators efficiently and intelligently select which samples to label. Our results show that a dataset constructed using effective active learning techniques requires less than half the amount of labelled data to achieve the same performance as a dataset constructed using random sampling.

Utah FORGE: Reports on Northwestern Nevada Well Doublet Drilling and Testing by Fervo Energy

These reports review Fervo Energy's construction of a commercial enhanced geothermal system (EGS). Fervo has qualified full functionality of the system through production testing at commercially relevant operating conditions. The project site is located in a nearfield setting adjacent to an operating geothermal power station in north-central Nevada and is designed to deliver an uplift in high-temperature geothermal flow rates to increase the power capacity at the facility. The project involved drilling a first-of-a-kind EGS horizontal doublet well system consisting of an injection and production well pair within a high-temperature hard-rock geothermal formation. The lithology of the target reservoir is characterized as a mixed metasedimentary and igneous formation, comprised of phyllite, quartzite, diorite, and granodiorite, representative of the geology across the most prospective geothermal areas throughout the western US. This work included a 16-stage plug-and-perforate stimulation treatment which was the first of its kind in a high-temperature mixed metasedimentary and granitic formation in a fully horizontal doublet geothermal well.

Bench Testing Data and Report for an Early Prototype Pitch Resonator WEC

This dataset encompasses data and documentation from bench tests conducted on an early prototype of a "pitch resonator" wave energy converter (WEC). The testing aimed to validate numerical models and reduce risks associated with the pitch resonator concept, which is designed to convert the pitching and rolling motions of a buoy into electrical power. The project's goal is to provide supplementary power, in the range of 10-100 watts, to the National Science Foundation's Ocean Observatories Initiative Pioneer Array. Two distinct testing phases are documented: one using a single degree of freedom (1DOF) test rig, and another employing a six degree of freedom (6DOF) Stewart platform, known as the Large Amplitude Motion Platform (LAMP). These tests assessed various factors, such as system performance in different motion scenarios, the torque exerted by wave forces, and the impact of mounting configurations. The dataset includes raw test data in MATLAB (.mat) format, detailed metadata, and a report describing the experimental procedures and preliminary findings.

Data from: "A guide to using GitHub for developing and versioning data standards and reporting formats"

These data are the results of a systematic review that investigated how data standards and reporting formats are documented on the version control platform GitHub. Our systematic review identified 32 data standards in earth science, environmental science, and ecology that use GitHub for version control of data standard documents. In our analysis, we characterized the documents and content within each of the 32 GitHub repositories to identify common practices for groups that version control their documents on GitHub.In this data package, there are 8 CSV files that contain data that we characterized from each repository, according to the location within the repository. For example, in 'readme_pages.csv' we characterize the content that appears across the 32 GitHub repositories included in our systematic review. Each of the 8 CSV files has an associated data dictionary file (names appended with '_dd.csv' and here we describe each content category within CSV files. There is one file-level metadata file (flmd.csv) that provides a description of each file within the data package.
