Corrective Action Decision Document/Corrective Action Plan for Corrective Action Units 101 and 102: Central and Western Pahute Mesa Nevada National Security Site, Nevada (Rev. 1)

This corrective action decision document (CADD)/corrective action plan (CAP) has been prepared for the Central and Western Pahute Mesa (PM) Corrective Action Units (CAUs) 101 and 102. Corrective action is needed because underground radioactive contamination exists in these areas resulting from historic underground nuclear testing. The PM CAUs are located in the northwestern portion of the Nevada National Security Site and comprise 82 corrective action sites. A total of 85 underground nuclear detonations took place within the area covered by the PM CAUs between 1965 and 1992 and resulted in the release of radionuclides (RNs) in the subsurface in the vicinity of the test cavities.

This revision was prepared to document changes to the data-collection activities. Specifically, the drilling and completion of the wells have been revised, and enhancements to the surface geophysics measurements have been included. Corrective action is needed because underground radioactive contamination exists in these areas resulting from historic underground nuclear testing. The PM CAUs are located in the northwestern portion of the Nevada National Security Site and comprise 82 corrective action sites. A total of 85 underground nuclear detonations took place within the area covered by the PM CAUs between 1965 and 1992 and resulted in the release of radionuclides in the subsurface in the vicinity of the test cavities.

Corrective Action Decision Document/Closure Report for Corrective Action Unit 542: Disposal Holes, Nevada Test Site, Nevada, Revision 0, with ROTC 1-2

The purpose of this Corrective Action Decision Document/Closure Report is to provide justification and documentation supporting the recommendation for closure of CAU 542 with no further corrective action. To achieve this, corrective action investigation (CAI) activities were performed from July 19 through August 25, 2006, as set forth in the Corrective Action Investigation Plan for Corrective Action Unit 542: Disposal Holes. The purpose of the CAI was to fulfill the following data needs as defined during the data quality objective (DQO) process: Determine whether contaminants of concern (COCs) are present. If COCs are present, determine the nature and extent. Provide sufficient information and data to complete appropriate corrective actions.

Streamlined Approach for Environmental Restoration (SAFER) Plan for Corrective Action Unit 114: Area 25 EMAD Facility Nevada National Security Site, Nevada

This Streamlined Approach for Environmental Restoration (SAFER) Plan addresses the actions needed to achieve closure for Corrective Action Unit (CAU) 114, Area 25 EMAD Facility, identified in the Federal Facility Agreement and Consent Order (FFACO). CAU 114 comprises the following corrective action sites (CASs) located in Area 25 of the Nevada National Security Site: • 25-41-03, EMAD Facility (Building 3900) • 25-99-23, Manned Control Car (MCC) and Engine Installation Vehicle (EIV) • 25-33-05, Building 3901, Engine Transport System Maintenance Building (Train Shed) This plan provides the methodology for field activities needed to gather the necessary information for closing CAU 114. There is sufficient information and process knowledge from historical documentation and investigations of similar sites regarding the expected nature and extent of potential contaminants to recommend closure of CAU 114 using the SAFER process. Additional information will be obtained by conducting a field investigation before selecting the appropriate corrective actions for CAU 114. It is anticipated that the results of the field investigation and implementation of corrective actions will support a defensible recommendation that no further corrective action is necessary. The purpose of the corrective action investigation will be to document and verify the adequacy of existing information; to affirm the decision for either clean closure, closure in place, or no further action; and to provide sufficient data to implement the corrective action. The actual corrective action selected will be based on characterization activities implemented under this SAFER Plan. If specific conditions or findings fall outside the bounds of the conceptual site model, such as an unanticipated release, the Nevada Division of Environmental Protection (NDEP) will be consulted to determine the path forward before proceeding. Upon completion of SAFER activities, a closure report (CR) will be prepared and submitted to NDEP for review and approval. The schedule for completion of the CR will be established in consultation with NDEP.

Streamlined Approach for Environmental Restoration (SAFER) Plan for Corrective Action Unit 578: Miscellaneous Inactive Sites Nevada National Security Site, Nevada, (Rev. 0)

This Streamlined Approach for Environmental Restoration (SAFER) Plan addresses the actions needed to achieve closure for Corrective Action Unit (CAU) 578, Miscellaneous Inactive Sites, identified in the Federal Facility Agreement and Consent Order (FFACO). CAU 578 comprises the following 11 corrective action sites (CASs) located in Areas 2, 3, 4, 6, 7, 12, and 23 of the Nevada National Security Site: • 02-26-01, Lead (Concrete Box w/Lining) • 02-26-07, Gun Turret w/Lead in Barrel • 03-16-01, U-3auS Disposal Site • 04-26-02, Lead on Instrument Bunker • 06-99-07, Sump • 07-99-01, Miscellaneous Debris • 12-02-11, UST-P-2 • 12-06-01, Muckpile • 12-06-04, Muckpile • 23-12-01, Boiler (2) • 23-99-04, Salvage Yard This plan provides the methodology for field activities needed to gather the necessary information for closing each CAS. There is sufficient information and process knowledge from historical documentation and investigations of similar sites regarding the expected nature and extent of potential contaminants to recommend closure of CAU 578 using the SAFER process. Additional information will be obtained by conducting a field investigation before selecting the appropriate corrective action for each CAS. The results of the field investigation will support a defensible recommendation that no further corrective action is necessary. This will be presented in a closure report that will be prepared and submitted to the Nevada Division of Environmental Protection (NDEP) for review and approval.

Closure Report for Corrective Action Unit 572: Test Cell C Ancillary Building and Structures, Nevada National Security Site, Nevada, Revision 0

The purpose of this CR is to provide documentation supporting the completed corrective actions and data confirming that the closure objectives for CAU 572 were met. To achieve this, the following actions were performed: • Corrective action investigation (CAI) activities were performed from August 2020 through May 2023, as set forth in the SAFER Plan for CAU 572; and in accordance with the Soils Activity Quality Assurance Plan, which establishes requirements, technical planning, and general quality practices. • Corrective actions were completed during decontamination and demolition (D&D) and disposal activities, and were performed from August 2022 through September 2025.

Closure Report for Corrective Action Unit 116: Area 25 Test Cell C Facility, Nevada National Security Site, Nevada with ROTC-1

Just adding new file which includes CR plus new ROTC 1 and update the metadata to the following: This Closure Report (CR) presents information supporting closure of Corrective Action Unit (CAU) 116, Area 25 Test Cell C Facility. This CR complies with the requirements of the Federal Facility Agreement and Consent Order (FFACO) that was agreed to by the State of Nevada; the U.S. Department of Energy (DOE), Environmental Management; the U.S. Department of Defense; and DOE, Legacy Management (FFACO, 1996 [as amended March 2010]). CAU 116 consists of the following two Corrective Action Sites (CASs), located in Area 25 of the Nevada National Security Site: (1) CAS 25-23-20, Nuclear Furnace Piping and (2) CAS 25-41-05, Test Cell C Facility. CAS 25-41-05 consisted of Building 3210 and the attached concrete shield wall. CAS 25-23-20 consisted of the nuclear furnace piping and tanks. Closure activities began in January 2007 and were completed in August 2011. Activities were conducted according to Revision 1 of the Streamlined Approach for Environmental Restoration Plan for CAU 116 (U.S. Department of Energy, National Nuclear Security Administration Nevada Site Office [NNSA/NSO], 2008). This CR provides documentation supporting the completed corrective actions and provides data confirming that closure objectives for CAU 116 were met. Site characterization data and process knowledge indicated that surface areas were radiologically contaminated above release limits and that regulated and/or hazardous wastes were present in the facility. The Record of Technical Change 1 updated the use restriction information.

Underground Test Area (UGTA) Sampling Plan for Corrective Action Units 101 and 102: Central and Western Pahute Mesa, Nevada National Security Site, Nevada, Revision 2

The purpose of this groundwater sampling plan (referred to herein as “the Plan”) is to describe the approach for collecting and analyzing groundwater samples to meet the objectives of the U.S. Department of Energy (DOE), Environmental Management (EM) Nevada Program’s Underground Test Area (UGTA) Activity. The Plan is focused on the Central and Western Pahute Mesa corrective action units (CAUs 101 and 102), which are currently in the Corrective Action Decision Document (CADD)/Corrective Action Plan (CAP) stage of the Federal Facility Agreement and Consent Order (FFACO) UGTA Strategy. The remaining UGTA CAUs are in the final stage of the UGTA strategy (Closure Report [CR] stage), and monitoring requirements for these CAUs are described in the Federal Facility Agreement and Consent Order Nevada National Security Site Use Restriction Management Plan (DOE/EMNV, 2024a). The Plan supersedes the Nevada National Security Site Integrated Groundwater Sampling Plan and its ensuing Record of Technical Change (ROTC) (DOE/EMNV, 2019b).

Non-Resource Conservation and Recovery Act Corrective Action Unit (CAU) Post-Closure Inspection Report, Nevada National Security Site, Nevada For Calendar Year 2025

This report includes visual inspection results, descriptions of maintenance and repair activities, and recommendations for CY 2025 for use restrictions (URs) at corrective action sites (CASs) located on the Nevada National Security Site and on the Nevada Test and Training Range (NTTR) that are accessed through the NNSS Main Gate. This document is arranged by project activity (i.e., Industrial Sites, Soils, and Defense Threat Reduction Agency [DTRA]); then by corrective action unit (CAU); and then by CAS. Post-closure UR inspections are performed on an annual basis, unless as noted in text. Post-closure UR inspections verify the condition of physical controls required by each UR, which may include the following activities: • Visual verification of the legibility of signs • Visual inspection of fencing, signs, monuments, and/or markers for indications of wear • Visual inspection of soil covers for indications of subsidence, erosion, or unauthorized use Deficiencies in UR requirements that were identified during CY 2024 inspections and not addressed in the CY 2024 report are addressed in this CY 2025 report.

2020 Postclosure Groundwater Monitoring and Inspection Report, Central Nevada Test Area, Subsurface Corrective Action Unit 443

This report presents the groundwater monitoring data collected by the U.S. Department of Energy (DOE) Office of Legacy Management (LM) from the Central Nevada Test Area (CNTA), Nevada, Site, Subsurface Corrective Action Unit (CAU) 443 in Nye County, Nevada (Figure 1). The CNTA is the site of an underground nuclear test in 1968 that resulted in residual contamination near the detonation depth of 3200 feet (ft); the contamination requires long-term monitoring. Responsibility for the environmental restoration and long-term monitoring was transferred from DOE’s National Nuclear Security Administration, Nevada Field Office, to LM on October 1, 2006. The environmental restoration and site closure process were completed in 2015 in accordance with the amended 1996 Nevada Federal Facility Agreement and Consent Order (FFACO) (State of Nevada et al. 1996, as amended) and all applicable Nevada Division of Environmental Protection (NDEP) policies and regulations. The Closure Report, Central Nevada Test Area, Subsurface Corrective Action, Unit 443 (DOE 2018), also called the Closure Report, originally was completed in January 2016 and revised in October 2018; it describes LM’s plan for long-term postclosure monitoring. This includes monitoring of the radioisotopes of interest and water elevations, inspecting the site and maintaining the institutional controls (ICs), evaluating and reporting data, and documenting the site’s records and data management processes (DOE 2018).

Groundwater Flow and Contaminant Transport Model of Corrective Action Units 101 and 102: Central and Western Pahute Mesa, Nye County, Nevada (Rev. 1)

This document presents the groundwater flow and transport models along with simulation forecasts needed for the purpose of developing the ensemble of contaminant boundaries (CBs) for the Central and Western Pahute Mesa (PM) corrective action units (CAUs) 101 and 102. The PM CAUs are areas of the Nevada National Security Site (NNSS) that were historically used for underground nuclear testing.

Corrective Action Unit 98: Frenchman Flat Five-Year Evaluation Nevada National Security Site, Nevada (Revision 1)

The Corrective Action Unit (CAU) 98, Frenchman Flat (FF) Closure Report (CR) (NNSA/NFO, 2016) and its ensuing Records of Technical Change (ROTCs) (compiled in DOE/EMNV, 2019) required the U.S. Department of Energy (DOE), Environmental Management (EM) Nevada Program’s Underground Test Area (UGTA) Activity to collect data for five years to establish triggers for the monitoring network and analyze the monitoring points for viability in long-term monitoring. This document provides the results of the five-year evaluation and presents recommendations for future monitoring of CAU 98.
