Review of the Constellation Level II Safety, Reliability, and Quality Assurance (SR&QA) Requirements Documents during Participation in the Constellation Level II SR&QA Forum

At the request of the Exploration Systems Mission Directorate (ESMD) and the Constellation Program (CxP) Safety, Reliability; and Quality Assurance (SR&QA) Requirements Director, the NASA Engineering and Safety Center (NESC) participated in the Cx SR&QA Requirements forum. The Requirements Forum was held June 24-26; 2008, at GRC's Plum Brook Facility. The forums purpose was to gather all stakeholders into a focused meeting to help complete the process of refining the CxP to refine its Level II SR&QA requirements or defining project-specific requirements tailoring. Element prime contractors had raised specific questions about the wording and intent of many requirements in areas they felt were driving costs without adding commensurate value. NESC was asked to provide an independent and thorough review of requirements that contractors believed were driving Program costs, by active participation in the forum. CMS Outer Tracker Module QA

The High Luminosity Large Hadron Collider is a planned upgrade to the LHC that will allow it to take up to 7 times as much data. Upgrades to all detector systems, including the Outer Tracker, are necessary to allow them to handle the increased radiation and pileup. Fermilab has been tasked with assembling modules for the Outer Tracker system. This presentation discusses the QA tests performed on these modules and the integration procedure required to ensure all modules work in sync. The QA tests include sensor IV curves, MaPSA testing, visual inspection, skeleton testing, and module testing. There are additional tests that can only be performed on integrated modules that are working in sync, and developing an understanding of the timing and latency of the modules is necessary to perform this synchronization.

Modified aerospace R&QA method for wind turbines

This paper describes the Safety, Reliability and Quality Assurance (SR&QA) approach developed for the first large wind turbine generator project, MOD-OA. The SR&QA approach to be used had to assure that the machine would not be hazardous, would operate unattended on a utility grid, would demonstrate reliable operation, and would help establish the quality assurance and maintainability requirements for wind turbine projects. The final approach consisted of a modified Failure Modes and Effects Analysis (FMEA) during the design phase, minimal hardware inspections during parts fabrication, and three documents to control activities during machine construction and operation.

Hardness-Strength Correlations in Zr(Cu)-Based BMG Alloys for QA Purposes

Quality assurance (QA) of bulk metallic glass (BMG) components would be expedited if hardness testing could provide a ready estimate of material strength. Consequently, a compilation of published data for Zr(Cu)-based alloys on the relationship between Vickers hardness (HV) and tensile/compressive strength (σ) is presented. An assessment reveals that the majority of data are confined within the hardness range of HV = 4.4−6.2 GPa and the strength range of σ = 1.4−2.3 GPa. Assuming a linear relationship, the statistical average of the hardness-strength correlation for BMGs (HV/σ = 2.87 ± 0.35) is very close to that established for non-austenitic steels (HV/σ = 2.86 ± 0.05). Unfortunately, the standard deviation of the BMG data is too broad for any type of correlation to be exploited for QA purposes. However, the narrow hardness range suggests that significant departures from the fully amorphous state resulting from processing variations may be detectable via indentation testing. Therefore, the relationship between hardness and volume/distribution of crystallinity for a specific alloy system warrants further investigation.

Implementation of R & QA practices in Research and Development programs

DOE has established a number of broad programs aimed at reducing fuel consumption. Several programs address the R&D of ground transportation propulsion alternatives to the conventional spark-ignition engine. NASA Lewis is responsible for managing the effort between the Government and industry teams involving American and foreign companies. Thus, existing NASA SR&QA procedure were modified/adapted to these R&D programs and implemented to assure that the test hardware design intent was met, the hardware was not hazardous to personnel, it would demonstrate reliable operation, and it would help establish the future R&D quality assurance and maintainability requirements. This successful low-cost approach might be applicable to other similar projects.

International collaboration leads to new Aten asteroid discovery, 1984 QA

A new earth-crossing asteroid of the Aten type (orbit smaller than earth's) was discovered on August 30, 1984 at Palomar. The asteroid, designated 1984 QA, is the fourth known member of the Aten type and the first discovered since 1978. The discovery of this object was the highlight of a collaboration between the JPL Asteroid Search Team and an ESA scientist, and demonstrates one aspect of an ongoing joint search effort with other Schmidt observers.

Hardware QA: Preparing and Certifying a Rover to Land on the Surface of Mars

The Mars Science Laboratory Project presented multiple challenges of complexity for hardware QA personnel. Technical complexity was a major challenge. This is a one-of-kind never-before-been-built Spacecraft with a nuclear-powered Rover the size of a small SUV. It features a never-before-been-used Skycrane landing system featuring a Descent Stage which is a complex spacecraft within itself. Equally complex was figuring out how to support multiple activities at multiple facilities in this country and around the world at the same time.

Application of Flex-QA Arrays in HTS Magnet Testing

Flexible PCB quench antennas have been very useful in providing high-quality high-resolution data in low temperature superconducting magnet tests. Similar multi-sensor arrays have been employed recently to cover a high temperature superconductor magnet tested at FNAL. In the present work, data taking conditions and magnet features to support the analysis framework are discussed. Then observations made during complete magnet powering cycles are described and analysis of quench antenna data are presented. Based on results, improvements to instrumentation and data taking are debated. Views on the future of flexible quench antenna sensors for HTS magnet diagnostics and operational support are shared.

Summary of Assessments Performed in FY 20 by SNL QA POC for FY 19 SNL NTRD Milestone Deliverables

The Sandia National Laboratories (SNL) staff is meeting the requirements of Nuclear Fuel Cycle and Supply Chain (NFCSC) Quality Assurance Program Document (QAPD). Each of the NFCSC FY 19 packages for SNL were reviewed. Minor errors were identified with regard to one package for the previous version of this report. Those are being rectified and future instances will indicate the appropriate entries. Aside from the minor discrepancies noted, there were no quality assurance findings this fiscal year for the NFCSC program. In particular, all of the Quality Rigor Level (QRL) level assignments were confirmed to be appropriate.

Summary of Assessments Performed in FY 20 by SNL QA POC for FY 19 SNL SFWD Milestone Deliverables

The Sandia National Laboratories (SNL) staff is meeting the requirements of the Nuclear Fuel Cycle and Supply Chain (NFCSC) Quality Assurance Program Document (QAPD)1. Each of the 46 SFWD FY 19 packages for SNL were reviewed. Six of the 46 packages had incorrect QRL categories, but technical reviews were always found to be appropriate. No major corrective actions are assigned, but recommendations have been made to adjust the identified QRL items. Additional and minor PICS:NE checkbox errors are recognized. Future training will be geared to ensure proper QRL categorizations and other check box entries in future cases

The Sandia National Laboratories (SNL) staff is meeting the requirements of the Nuclear Fuel Cycle and Supply Chain (NFCSC) Quality Assurance Program Document (QAPD). Each of the 5 NFCSC FY 20 packages for SNL were reviewed. None of the 5 packages had incorrect QRL categories. No major corrective actions are assigned. Additional and minor PICS:NE checkbox errors may exist, but none were identified in this assessment. Training is now also geared to encourage WPMs to take credit for optional technical reviews by considering setting the QRL level to 3. This is because when the level is set to 4, the PICS:NE system does not provide fields for review information. Finally, training in the NFCSC QAPD has been delivered as always to WPMs.

PTSFA QA Review of Container Procurements: Information Use Procedure

The purpose of this Packaging and Transportation Safety Functional Area (PTSFA) procedure is to describe the process in which the PTSFA Quality Assurance Engineer (QAE) or designee reviews and approves the documentation associated with the procurements of containers to be used for onsite transfer or off-site transportation of hazardous materials and waste.
