Patterns of Care, Tolerability, and Safety of the First Cohort of Patients Treated on a Novel High-Field MR-Linac Within the MOMENTUM Study: Initial Results From a Prospective Multi-Institutional Registry

High-field magnetic resonance-linear accelerators (MR-Linacs), linear accelerators combined with a diagnostic magnetic resonance imaging (MRI) scanner and online adaptive workflow, potentially give rise to novel online anatomic and response adaptive radiation therapy paradigms. The first high-field (1.5T) MR-Linac received regulatory approval in late 2018, and little is known about clinical use, patient tolerability of daily high-field MRI, and toxicity of treatments. Herein we report the initial experience within the MOMENTUM Study (NCT04075305), a prospective international registry of the MR-Linac Consortium.

Results of a Multi-institutional Phase 2 Clinical Trial for 4DCT-ventilation Functional Avoidance Thoracic Radiation Therapy

Radiation pneumonitis remains a major limitation in the radiation therapy treatment of patients with lung cancer. Functional avoidance radiation therapy uses functional imaging to reduce pulmonary toxic effects by designing radiation therapy plans that reduce doses to functional regions of the lung. Lung functional imaging has been developed that uses 4-dimensional computed tomography (4DCT) imaging to calculate 4DCT-based lung ventilation (4DCT-ventilation). A phase 2 multicenter study was initiated to evaluate 4DCT-ventilation functional avoidance radiation therapy. The study hypothesis was that functional avoidance radiation therapy could reduce the rate of grade ≥2 radiation pneumonitis to 12% compared with a 25% historical rate, with the trial being positive if ≤16.4% of patients experienced grade ≥2 pneumonitis.

First multi-institutional systematic comparison of the neutron ambient dose equivalent produced by proton therapy systems

Objective. Isochronous cyclotrons, synchrocyclotrons, and synchrotrons are used to accelerate protons for proton therapy. An accurate measurement of neutron doses generated by these accelerators and associated delivery systems and its clinical relevance requires systematic protocols and proper neutron dosimetry for a meaningful assessment. We present the first comprehensive comparison of neutron ambient dose equivalent (H*(10)) produced by clinically operational proton therapy systems. Approach. Treatment plans with 10 cm modulation-depth and ranges of 10 cm (R10M10) and 25 cm (R25M10) were created to cover a 10 × 10 × 10 cm 3 water target. The pencil beam scanning proton therapy machines studied were: two gantry-mounted synchrocyclotrons (Hyperscan, Mevion, half-gantry), two isochronous cyclotrons (ProBeam, Varian, full-gantry), one isochronous cyclotron (Proteus, IBA, full-gantry), and two synchrotrons (PROBEAT, Hitachi, full- and half-gantry). Proton beams were delivered to 30 × 30 × 40 cm 3 plastic water phantoms. WENDI-II and LUPIN-BF3-NP neutron rem-meters were positioned at three angles (0°, 45°, 90°) relative to the beam direction to measure the neutron H*(10) at distances between 50–300 cm from the isocenter. Main results. H*(10) showed dependence on beam energy, machine type, and measurement location. The highest reading was for the gantry-mounted synchrocyclotron, whereas other systems produced approximately comparable neutron doses. In all cases, the H*(10) reduced with distance from the isocenter. The H*(10) drop at 2 m distance compared to that at 0.5 m was a factor of ∼5 for the gantry-mounted synchrocyclotron whereas in other systems the decrease was a factor of 10. The WENDI-II device suffered from dead-time-associated under-estimation of the dose by a factor of ∼2–3 under the synchrocyclotron beam due to its high dose-per-pulse. However, WENDI-II and LUPIN-BF3-NP results were within reasonable agreement in isochronous cyclotron and synchrotron beams, indicating that both devices are suitable for those systems. Significance. Neutron H*(10) is dependent on various parameters including beam energy, measurement location, as well as machine design. Caution must be exercised in choosing the appropriate neutron-dose-measurement device to be used for low-duty-factor, particularly in high-instantaneous-rate proton delivery systems. By delivering the same volumetric proton dose across different machines, this work provides a benchmark for inter-system comparisons and serves as a foundation for future studies.

Using a Multi‐Institutional Ensemble of Watershed Models to Assess Agricultural Conservation Effectiveness in a Future Climate

This study investigates the combined impacts of climate change and agricultural conservation on the magnitude and uncertainty of nutrient loadings in the Maumee River Watershed, the second-largest watershed of the Laurentian Great Lakes. Two scenarios — baseline agricultural management and increased agricultural conservation — were assessed using an ensemble of five Soil and Water Assessment Tools driven by six climate models. The increased conservation scenario included raising conservation adoption rates from a baseline of existing conservation practices to feasible rates in the near future based on farmer surveys. This increased adoption of winter cover crops on 6%–10% to 60% of cultivated cropland; subsurface placement of phosphorus fertilizers on 35%–60% to 68% of cultivated cropland; and buffer strips intercepting runoff from 29%–34% to 50% of cultivated cropland. Increased conservation resulted in statistically significant (p ≤ 0.05) reductions in annual loads of total phosphorus (41%), dissolved reactive phosphorus (18%), and total nitrogen (14%) under the highest emission climate scenario (RCP 8.5). While nutrient loads decreased with increased conservation relative to baseline management for all watershed models, different conclusions on the true effectiveness of conservation under climate change may be drawn if only one watershed model was used.

Multi-species outbreak of SARS-CoV-2 Delta variant in a zoological institution, with the detection in two new families of carnivores

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has a worldwide distribution in humans and many other mammalian species. In late September 2021, 12 animals maintained by the Chicago Zoological Society's Brookfield Zoo were observed with variable clinical signs. The Delta variant of SARS-CoV-2 was detected in faeces and nasal swabs by qRT-PCR, including the first detection in animals from the families Procyonidae and Viverridae. Test positivity rate was 12.5% for 35 animals tested. All animals had been vaccinated with at least one dose of a recombinant vaccine designed for animals and all recovered with variable supportive treatment. Sequence analysis showed that six zoo animal strains were closely correlated with 18 human SARS-CoV-2 strains, suggestive of potential human-to-animal transmission events. This report documents the expanding host range of COVID-19 during the ongoing pandemic.

The LCLStream Ecosystem for Multi-Institutional Dataset Exploration

We describe a new end-to-end experimental data streaming framework designed from the ground up to support new types of applications – AI training, extremely high-rate X-ray time-of-flight analysis, crystal structure determination with distributed processing, and custom data science applications and visualizers yet to be created. Throughout, we use design choices merging cloud microservices with traditional HPC batch execution models for security and flexibility. This project makes a unique contribution to the DOE Integrated Research Infrastructure (IRI) landscape. By creating a flexible, API-driven data request service, we address a significant need for high-speed data streaming sources for the X-ray science data analysis community. With the combination of data request API, mutual authentication web security framework, job queue system, high-rate data buffer, and complementary nature to facility infrastructure, the LCLStreamer framework has prototyped and implemented several new paradigms critical for future generation experiments.

Forecasting for ESCAPE: A Multi-Institution Hybrid Forecasting and Nowcasting Operation for Sea-Breeze Convection Supporting a Ground-Based and Airborne Field Campaign

The Experiment of Sea-Breeze Convection, Aerosols, Precipitation and Environment (ESCAPE) field project deployed two aircraft and ground-based assets in the vicinity of Houston, Texas, between 27 May and 2 July 2022, examining how meteorological conditions, dynamics, and aerosols control the initiation, early growth stage, and evolution of coastal convective clouds. To ensure that airborne- and ground-based assets were deployed appropriately, a forecasting and nowcasting team was formed. Daily forecasts guided real-time decision-making by assessing synoptic weather conditions, environmental aerosol, and a variety of atmospheric modeling data to assign a probability for meeting specific ESCAPE campaign objectives. During the research flights, a small team of forecasters provided “nowcasting” support by analyzing radar, satellite, and new model data in real time. The nowcasting team proved invaluable to the campaign operation, as sometimes changing environmental conditions affected, for example, the timing of convective initiation. In addition to the success of the forecasting and nowcasting teams, the ESCAPE campaign offered a unique “testbed” opportunity where in-person and virtual support both contributed to campaign objectives. The forecasting and nowcasting teams were each composed of new and experienced forecasters alike, where new forecasters were given invaluable experience that would otherwise be difficult to attain. Both teams received training on forecast models, map analysis, Hybrid Single-Particle Lagrangian Integrated Trajectory model (HYSPLIT), and thermodynamic sounding analysis before the beginning of the campaign. In this article, the ESCAPE forecasting and nowcasting teams reflect on these experiences, providing potentially useful advice for future field campaigns requiring forecasting and nowcasting support in a hybrid virtual/in-person framework.

Protect Your Family: Reduce Contamination at Home: A Summary of a Study Conducted by the National Institute for Occupational Safety and Health

Congress enacted the Health Insurance Portability and Accountability Act of 1996 (HIPAA) without including specific provisions for protecting the privacy of healthrelated information. The law gave Congress three years to enact such requirements and provided that if it failed to do so the Secretary of the Department of Health and Human Services (DHHS) was obligated to promulgate regulations to implement the statutory standards. When Congress failed to meet the deadline, the obligation to issue privacy regulations passed to DHHS. DHHS received over 52,000 comments on the proposed regulations. Responses to the comments and the final rule were published in the Federal Register on December 28, 2000. The cost of implementing the regulations over a ten-year period was estimated at $17.6 billion by DHHS. Industry estimates the cost at five times that amount.
