Reducing COVID-19 Transmission in Homes

Owners and renters all over the world are being encouraged to stay home as much as possible to mitigate the spread of the SARS CoV-2 virus and the COVID-19 disease. It is prudent to take reasonable precautions at home to decrease risks during the pandemic because most COVID-19 transmission happens in the home (see “learn more”). This is particularly true for households with infected or sensitive members. Furthermore, because people infected with COVID-19 can be asymptomatic you may have an infected person in your home for several days before you know it.

Evaluation and optimization for the decontamination/sterilization of personal protective equipment using a novel ozone chamber (CRADA Final Report)

Personal protective equipment slows the spread of the SARS-Cov-2 virus and protects first responders and the general population. However, there is a significant and well-publicized shortage of personal protective equipment for healthcare workers. Treatment with ozone may be a compelling, affordable, and scalable method to rapidly decontaminate personal protective equipment but more work is needed to verify these claims. Together with colleagues at the University of Nevada Reno and FATHHOME we helped design the experiments that evaluated the virucidal efficacy of FATHHOME’s device on N95 face masks and face shields. The results of our study were published in the journal Pathogens.

Final CTAP Report

Sandia will provide technical assistance to Helpful Engineering to develop and test the Universal Citizen Protection Device (UCPD) which is a UV-based filter-less PPE concept that aims to keep the Sars-CoV-2 virus out of eyes, nose and mouth with a 99%+ reliability. The heart of the device would be a concealed UV Chamber that decontaminates all air going in and out of the PPE. Helpful Engineering’s goal is to build this device to be reusable and cost less than $100 to construct and can be worn for 8 hours. The UCPD is an open source project, and once developed, prototyped, tested and approved, it will be shared with interested manufacturers globally.

COVID-19 Lead Time: Evaluating the timeliness and reliability of reported COVID-19 cases and hospitalizations as leading indicators of hospitalizations and death in the US

The COVID-19 pandemic prompted a reliance on real-time data sources to understand the global spread and impact of the SARS-CoV-2 virus. Reported cases were presumed leading indicators for hospitalizations, while hospitalizations were considered predictive of deaths. However, studies have questioned the consistency of reported cases as leading indicators. This study systematically assesses the reliability of United States (US) reported cases and hospitalizations as leading indicators for hospitalizations and deaths respectively, examining the first 2.5 years of the pandemic (January 2020 - June 2022) across different phases of the pandemic and states. Using correlation analysis, population data, and forecasting accuracy measures, we investigate the temporal relationships and identify possible determinants of lead time variability. Notably, we found that the average lead time between reported cases and hospitalizations across US states is relatively short at 1.76 days, implying that reported cases might not be as effective a leading indicator for hospitalizations as previously believed. Populations with higher comorbidity burdens, such as proportion of smokers, are expected on average to have shorter lead times, possibly due to shorter time to hospitalization among these vulnerable populations.

Threat Agnostic Virulence Assessment of Pathogens

Virulence assessment of new, emerging, and engineered pathogens is critical to mounting an appropriate response to a biothreat agent. The capacity of the pathogen to colonize human and harm tissues must be characterized to understand pathogenicity pathways and optimize diagnosis and treatment of resulting disease. Respiratory pathogens are of interest because they can have high transmissibility rates, as observed with the SARS-CoV-2 virus, the causative agent of Covid-19. Current technologies are insufficient to assess threats due to their reliance on systems with only one cell type and on sequencing the pathogen. However, it is known that sequence is not an accurate predictor of function, and sequencing can be unreliable for newly emerged or engineered pathogens. An ideal system would consist of relevant epithelial cell types and an assay sensitive enough to detect changes in host responses that do not rely on DNA sequencing. We chose a system consisting of host lung epithelial cells that can be used to assess the virulence of unknown respiratory pathogens. We interrogated pathogens using this model and assess features of pathogenicity. Our objective is to leverage PNNLs strengths in tissue engineering and proteomics capabilities to build a multiple reaction monitoring (MRM) or parallel reaction monitoring (PRM) liquid chromatography-tandem mass spectrometry assay for human host cell proteins whose abundance is influenced by infection. These responses can were then assessed for relative virulence using pathogen agnostic signatures. When confronted with a pathogen, cells activate dedicated signaling pathways, typically through phosphorylation of regulatory proteins and downstream activation of host cell networks.

Optimizing Layered Control Strategies for Reducing Exposure to SARS-CoV-2

While there is growing understanding of the predominance of the airborne mode of transmission of the SARS-CoV-2 virus, there is a lack of guidance on how to build an effective system of controls to mitigate transmission in enclosed spaces. Such a system integrates multiple hazard-specific control measures to both eliminate or replace a hazard, and safeguard individuals against potential exposure and infection. Controls are defined by the US Occupational Safety and Health Association (OSHA) as the use of engineering methods to reduce the level of hazard inside a confined space. The Hierarchy of Controls provides a framework through which a system of controls can be examined; identifying those which directly remove or replace a hazard as the most effective, and individual behavioral measures (e.g., masking or shielding) as least effective. Use of such a framework to quantify control effectiveness might allow for the promotion of spaces as meeting a certain threshold of safety, likely adding to the confidence of space users resulting in increases in various metrics such as sales, visits, or likes.

Reveal-CoV Diaagnostic Platform (Final Report)

LLNL designed and built a rapid, RT-LAMP-based molecular diagnostics platform as a potential tool to quickly diagnose COVID-19 in under one hour. This point-of-care testing approach involves an initial high temperature swab sample inactivation step followed by amplification of viral RNA using up to 5 control and pathogen-specific assays. Results are determined based on a discreet reaction color change from red to yellow but can also be determined using fluorescence detection. Testing of this prototype platform was conducted with synthetic viral RNA and dried, stabilized reagents. Buffer systems, swab selection, and assay stabilization formulations were evaluated for performance. Limits of detection were determined using RNA; however, testing was not performed with viable SARS-CoV-2 virus or clinical samples.

Whole-genome demography of COVID-19 virus during its pandemic period and on “panvalent” vaccine design

With over 16 million submitted genomic sequences, the SARS-CoV-2 (SC2) virus, the cause of the most recent worldwide COVID-19 pandemic, has become the most sequenced genome of all known viruses, revealing, for example, a vast number of expanding viral lineages. Since the pandemic phase appears to be over, we performed a retrospective re-examination of the demographic grouping pattern and their genomic characteristics during the entire pandemic period up to the peak of the last pandemic wave. For our study, we extracted from the NCBI only unique viral sequences and converted each sequence data to a relational vector, indicating the presence/absence of each variational event compared to a “reference” sequence. Our study revealed several genomic features that are unexpected or different from those of previous studies. For example, approximately 44,000 variants with unique sequences emerged during the pandemic period; they group into only four major viral-genomic groups and each has a set of mostly unique highly-conserved variant-genotypes (HCVGs); and a small set from the first (“ancestral”) group was inherited by the three (“descendant”) groups, suggesting that HCVGs in the next group may be predictable from the current group(s). Such a concept may be potentially important in designing “panvalent” vaccines against the current and future waves of viral infections.

Approaches for effective negative pressure isolation space control to minimize airborne transmission of contaminants in residential homes

Controlling airborne transmission of contaminants including respired viruses such as SARS-CoV-2 is necessary to protect occupants living in the same house with a contagious person. The effectiveness of interventions requiring minor efforts that create a negative pressure isolation zone (IZ) for a contagious person has yet to be systematically tested for residential homes. In this study, ASHRAE Standard 170, which offers guidance for negative pressure isolation space control in healthcare facilities, was used in developing practical and attainable recommendations for residential single-family homes. The relative effectiveness of several control strategies was evaluated through experimentally conducting 17 different test cases in a manufactured single-family house laboratory. These cases were designed based on various heating, ventilation and air-conditioning (HVAC) operating scenarios, intervention measures including closing the IZ door and/or sealing over supply and return air grilles in the IZ, and utilization of bathroom exhaust or portable window fans for pressure control. Four out of 17 test cases were identified as having the potential for strong containment with adequate depressurization in the IZ. The most effective IZ depressurization was achieved through continuously operating the exhaust fan in the bathroom attached to the IZ, by installing a portable window fan that extracted air out of the IZ, and a portable room air conditioner with the AC unit exhaust duct installed in one of the IZ windows.

Evolutionary history, potential intermediate animal host, and cross-species analyses of SARS-CoV-2

To investigate the evolutionary history of the recent outbreak of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in China, a total of 70 genomes of virus strains from China and elsewhere with sampling dates between 24 December 2019 and 3 February 2020 were analyzed. To explore the potential intermediate animal host of the SARS-CoV-2 virus, we reanalyzed virome data sets from pangolins and representative SARS-related coronaviruses isolates from bats, with particular attention paid to the spike glycoprotein gene. We performed phylogenetic, split network, transmission network, likelihood-mapping, and comparative analyses of the genomes. Additionally, based on Bayesian time-scaled phylogenetic analysis using the tip-dating method, we estimated the time to the most recent common ancestor and evolutionary rate of SARS-CoV-2, which ranged from 22 to 24 November 2019 and 1.19 to 1.31 × 10 -3 substitutions per site per year, respectively. Our results also revealed that the BetaCoV/bat/Yunnan/RaTG13/2013 virus was more similar to the SARS-CoV-2 virus than the coronavirus obtained from the two pangolin samples (SRR10168377 and SRR10168378). We also identified a unique peptide (PRRA) insertion in the human SARS-CoV-2 virus, which may be involved in the proteolytic cleavage of the spike protein by cellular proteases, and thus could impact host range and transmissibility. Interestingly, the coronavirus carried by pangolins did not have the RRAR motif. Therefore, we concluded that the human SARS-CoV-2 virus, which is responsible for the recent outbreak of COVID-19, did not come directly from pangolins.

Single-molecule 3D imaging of HIV cellular entry by liquid-phase electron tomography

Enveloped viruses, including human immunodeficiency virus (HIV) and SARS-CoV-2, target cells through membrane fusion process. The detailed understanding of the process is sought after for vaccine development but remains elusive due to current technique limitations for direct three-dimensional (3D) imaging of an individual virus during its viral entry. Recently, we developed a simple specimen preparation method for real-time imaging of metal dynamic liquid-vaper interface at nanometer resolution by transmission electron microscopy (TEM). Here, we extended this method to study biology sample through snapshot 3D structure of a single HIV (pseudo-typed with the envelope glycoprotein of vesicular stomatitis virus, VSV-G) at its intermediate stage of viral entry to HeLa cells in a liquid-phase environment. By individual-particle electron tomography (IPET), we found the viral surface release excess lipids with unbound viral spike proteins forming ~50-nm nanoparticles instead of merging cell membrane. Moreover, the spherical-shape shell formed by matrix proteins underneath the viral envelope does not disassemble into a cone shape right after fusion. Further, the snapshot 3D imaging of a single virus provides us a direct structure-based understanding of the viral entry mechanism, which can be used to examine other viruses to support the development of vaccines combatting the current ongoing pandemic.

Multivalency transforms SARS-CoV-2 antibodies into ultrapotent neutralizers

SARS-CoV-2, the virus responsible for COVID-19, has caused a global pandemic. Antibodies can be powerful biotherapeutics to fight viral infections. Here, we use the human apoferritin protomer as a modular subunit to drive oligomerization of antibody fragments and transform antibodies targeting SARS-CoV-2 into exceptionally potent neutralizers. Using this platform, half-maximal inhibitory concentration (IC 50 ) values as low as 9 × 10 - 14 M are achieved as a result of up to 10,000-fold potency enhancements compared to corresponding IgGs. Combination of three different antibody specificities and the fragment crystallizable (Fc) domain on a single multivalent molecule conferred the ability to overcome viral sequence variability together with outstanding potency and IgG-like bioavailability. The MULTi-specific, multi-Affinity antiBODY (Multabody or MB) platform thus uniquely leverages binding avidity together with multi-specificity to deliver ultrapotent and broad neutralizers against SARS-CoV-2. The modularity of the platform also makes it relevant for rapid evaluation against other infectious diseases of global health importance. Neutralizing antibodies are a promising therapeutic for SARS-CoV-2.
