Pivotal trial characteristics and types of endpoints used to support Food and Drug Administration rare disease drug approvals between 2013 and 2022

Background/aims Rare disease drug development faces unique challenges, such as genotypic and phenotypic heterogeneity within small patient populations and a lack of established outcome measures for conditions without previously successful drug development programs. These challenges complicate the process of selecting the appropriate trial endpoints and conducting clinical trials in rare diseases. In this descriptive study, we examined novel drug approvals for non-oncologic rare diseases by the U.S. Food and Drug Administration’s Center for Drug Evaluation and Research over the past decade and characterized key regulatory and trial design elements with a focus on the primary efficacy endpoint utilized as the basis of approval. Methods Using the Food and Drug Administration’s Data Analysis Search Host database, we identified novel new drug applications and biologics license applications with orphan drug designation that were approved between 2013 and 2022 for non-oncologic indications. From Food and Drug Administration review documents and other external databases, we examined characteristics of pivotal trials for the included drugs, such as therapeutic area, trial design, and type of primary efficacy endpoints. Differences in trial design elements associated with primary efficacy endpoint type were assessed such as randomization and blinding. Then, we summarized the primary efficacy endpoint types utilized in pivotal trials by therapeutic area, approval pathway, and whether the disease etiology is well defined. Results One hundred and seven drugs that met our inclusion criteria were approved between 2013 and 2022. Assessment of the 107 drug development programs identified 150 pivotal trials that were subsequently analyzed. The pivotal trials were mostly randomized (80%) and blinded (69.3%). Biomarkers (41.1%) and clinical outcomes (42.1%) were commonly utilized as primary efficacy endpoints. Analysis of the use of clinical trial design elements across trials that utilized biomarkers, clinical outcomes, or composite endpoints did not reveal statistically significant differences. The choice of primary efficacy endpoint varied by the drug’s therapeutic area, approval pathway, and whether the indicated disease etiology was well defined. For example, biomarkers were commonly selected as primary efficacy endpoints in hematology drug approvals (70.6%), whereas clinical outcomes were commonly selected in neurology drug approvals (69.6%). Further, if the disease etiology was well defined, biomarkers were more commonly used as primary efficacy endpoints in pivotal trials (44.7%) than if the disease etiology was not well defined (27.3%). Discussion In the past 10 years, numerous novel drugs have been approved to treat non-oncologic rare diseases in various therapeutic areas. To demonstrate their efficacy for regulatory approval, biomarkers and clinical outcomes were commonly utilized as primary efficacy endpoints. Biomarkers were not only frequently used as surrogate efficacy endpoints in accelerated approvals, but also in traditionally approved rare disease drugs. The choice of primary efficacy endpoints varied by therapeutic area, approval pathway, and understanding of disease etiology.

Utah FORGE: EGS Reservoir Produced Fluids Geochemistry 2022-2024

This dataset contains geochemical analyses of produced fluids from the Utah FORGE site, specifically from wells 16A(78)-32, 16B(78)-32, and 58-32, collected during various stimulation, flowback, and circulation tests conducted between 2022 and 2024. The data contains element concentrations, pH, and dissolved gas compositions. Geochemical analyses for 2022 and 2023 were performed at the Brigham Young University geochemistry laboratory, while the 2024 results were obtained from Thermochem. Dry gas samples were collected using a mini-separator attached to the single-phase production line between the wellhead and separator, where fluid was flashed to atmospheric pressure. Gas concentrations were recalculated to a single phase reservoir liquid based on heat and mass balance expressions. Additional contextual information, including interpretations of geochemical trends and reservoir behavior, is available in the included report from Simmons et al. (2025), which was presented at the Stanford Geothermal Workshop in February 2025.

2022 Bull E-Bike Pilot Program Study

In 2022, the City of Durham conducted an e-bike pilot program study to learn more about how electric bikes (e-bikes) could improve the transportation experience in the "Bull City" (a.k.a., Durham, North Carolina). The study used pedal-assist e-bikes, which feature an electric motor and battery to help power the bike. The motor amplifies the power behind each pedal stroke, augmenting the energy you put into the bike. #### Data Collection Agency The City of Durham's Transportation Department conducted the study. #### Survey Methodology Program participants used electric-assist e-bikes for at least 4 weeks between August and November 2022 in exchange for sharing information about their experiences, including tracking their travel via a smartphone app. In addition to the e-bike, participants received maintenance support along with a helmet, bike lock, and other accessories. Data collection was enabled by the [NLR OpenPATH platform](https://www.nlr.gov/transportation/openpath.html). #### Survey Records and Data Survey records include a total of 76 participants. The dataset contains 3 months of partially automated travel diaries, combining sensed and surveyed travel behavior data—patterns of multimodal, end-to-end, individual human mobility—as well as demographic and socioeconomic information from participants. The number of total trips was 6,488, the number of e-bike trips was 2,183, and the number of e-bike miles traveled was 5,450.

2022 Central California Travel Study

# 2022 Central California Travel Study The 2022 Central California Travel Study collected demographic and travel pattern information to better understand household travel behavior in the state’s San Joaquin Valley region. ## Data Collection Agency The Fresno Council of Governments conducted the household travel study in collaboration with metropolitan planning organizations in Fresno, Kern, Kings, Madera, Merced, San Joaquin, Stanislaus, and Tulare counties in California. ## Survey Methodology The two-part study consisted of a survey and a travel diary. The survey gathered data on household demographic composition and typical travel behaviors. The travel diary gathered individual travel data during a specified travel period for all members of a given household. ## Travel Diary Methods Households with smartphones were encouraged to complete their travel diaries using the rMove smartphone app for up to seven consecutive days. Households without smartphones or those unwilling to participate via smartphone completed their travel diaries online (using the web-version of rMove) or by calling the survey call center. These households reported travel for one day (Tuesday, Wednesday, or Thursday). ## Recruitment Sampling Methods The majority of the recruitment was conducted via address-based sampling, a type of probability sampling, with a focus on reaching county-level targets in collaboration with metropolitan planning organizations in the region. Supplemental sampling methods, primarily non-probability, were employed during all waves of data collection to improve survey representation. The supplemental sample included targeted outreach to hard-to-survey populations via transit rider email lists; local housing authorities; Nichols Research, a California-based market research firm; and the Ipsos™ KnowledgePanel, an online random probability panel. The travel diary gathered individual travel data during a specified travel period for all members of a given household. ## Travel Diary Methods Households with smartphones were encouraged to complete their travel diaries using the rMove smartphone app for up to seven consecutive days. Households without smartphones or those unwilling to participate via smartphone completed their travel diaries online (using the web-version of rMove) or by calling the survey call center. These households reported travel for one day (Tuesday, Wednesday, or Thursday). ## Recruitment Sampling Methods The majority of the recruitment was conducted via address-based sampling, a type of probability sampling, with a focus on reaching county-level targets in collaboration with metropolitan planning organizations in the region. Supplemental sampling methods, primarily non-probability, were employed during all waves of data collection to improve survey representation. The supplemental sample included targeted outreach to hard-to-survey populations via transit rider email lists; local housing authorities; Nichols Research, a California-based market research firm; and the Ipsos™ KnowledgePanel, an online random probability panel. See the documentation for more information about sampling and weighting. ## Survey Records, Data, and Documentation In total, 7,406 households completed 19,084 surveys representing 150,012 trips across 42,567 person-days.
