Independent Orbiter Assessment (IOA): Assessment of the life support and airlock support systems, volume 1

The results of the Independent Orbiter Assessment (IOA) of the Failure Modes and Effects Analysis (FMEA) and Critical Items List (CIL) are presented. The IOA effort first completed an analysis of the Life Support and Airlock Support Systems (LSS and ALSS) hardware, generating draft failure modes and potential critical items. To preserve independence, this analysis was accomplished without reliance upon the results contained within the NASA FMEA/CIL documentation. The IOA results were then compared to the NASA FMEA/CIL baseline with proposed Post 51-L updates included. The discrepancies were flagged for potential future resolution. This report documents the results of that comparison for the Orbiter LSS and ALSS hardware. The IOA product for the LSS and ALSS analysis consisted of 511 failure mode worksheets that resulted in 140 potential critical items. Comparison was made to the NASA baseline which consisted of 456 FMEAs and 101 CIL items. The IOA analysis identified 39 failure modes, 6 of which were classified as CIL items, for components not covered by the NASA FMEAs. It was recommended that these failure modes be added to the NASA FMEA baseline. The overall assessment produced agreement on all but 301 FMEAs which caused differences in 111 CIL items.

Design and Verification Guidelines for Vibroacoustic and Transient Environments

Design and verification guidelines for vibroacoustic and transient environments contain many basic methods that are common throughout the aerospace industry. However, there are some significant differences in methodology between NASA/MSFC and others - both government agencies and contractors. The purpose of this document is to provide the general guidelines used by the Component Analysis Branch, ED23, at MSFC, for the application of the vibroacoustic and transient technology to all launch vehicle and payload components and payload components and experiments managed by NASA/MSFC. This document is intended as a tool to be utilized by the MSFC program management and their contractors as a guide for the design and verification of flight hardware.

Ascent/Descent Software

The Ascent/Descent Software Suite has been used to support a variety of NASA Shuttle Program mission planning and analysis activities, such as range safety, on the Integrated Planning System (IPS) platform. The Ascent/Descent Software Suite, containing Ascent Flight Design (ASC)/Descent Flight Design (DESC) Configuration items (Cis), lifecycle documents, and data files used for shuttle ascent and entry modeling analysis and mission design, resides on IPS/Linux workstations. A list of tools in Navigation (NAV)/Prop Software Suite represents tool versions established during or after the IPS Equipment Rehost-3 project.

Debris/Ice/TPS assessment and integrated photographic analysis of Shuttle mission STS-67

A debris/ice/thermal protection system assessment and integrated photographic analysis was conducted for Shuttle mission STS-67. Debris inspections of the flight elements and launch pad were performed before and after launch. Icing conditions on the External Tank were assessed by the use of computer programs and infrared scanner data during cryogenic loading of the vehicle, followed by on-pad visual inspection. High speed photography of the launch was analyzed to identify ice/debris sources and evaluate potential vehicle damage and/or in flight anomalies. This report documents the ice/debris/thermal protection (TPS) conditions and integrated photographic analysis of Shuttle mission STS-67, and the resulting effect on the Space Shuttle Program.
