Optimal beam margins in linac-based VMAT stereotactic ablative body radiotherapy: a Pareto front analysis for liver metastases

We explored the Pareto fronts mathematical strategy to determine the optimal block margin and prescription isodose for stereotactic body radiotherapy (SBRT) treatments of liver metastases using the volumetric-modulated arc therapy (VMAT) technique. Three targets (planning target volumes [PTVs] = 20, 55, and 101 cc) were selected. A single fraction dose of 26 Gy was prescribed (prescription dose [PD]). VMAT plans were generated for 3 different beam energies. Pareto fronts based on (1) different multileaf collimator (MLC) block margin around PTV and (2) different prescription isodose lines (IDL) were produced. For each block margin, the greatest IDL fulfilling the criteria (95% of PTV reached 100%) was considered as providing the optimal clinical plan for PTV coverage. Liver D{sub mean}, V7Gy, and V12Gy were used against the PTV coverage to generate the fronts. Gradient indexes (GI and mGI), homogeneity index (HI), and healthy liver irradiation in terms of D{sub mean}, V7Gy, and V12Gy were calculated to compare different plans. In addition, each target was also optimized with a full-inverse planning engine to obtain a direct comparison with anatomy-based treatment planning system (TPS) results. About 900 plans were calculated to generate the fronts. GI and mGI show a U-shaped behavior as a function of beam margin with minimal values obtained with a +1 mm MLC margin. For these plans, the IDL ranges from 74% to 86%. GI and mGI show also a V-shaped behavior with respect to HI index, with minimum values at 1 mm for all metrics, independent of tumor dimensions and beam energy. Full-inversed optimized plans reported worse results with respect to Pareto plans. In conclusion, Pareto fronts provide a rigorous strategy to choose clinical optimal plans in SBRT treatments. We show that a 1-mm MLC block margin provides the best results with regard to healthy liver tissue irradiation and steepness of dose fallout.

Feed-forward alpha particle radiotherapy ablates androgen receptor-addicted prostate cancer

Human kallikrein peptidase 2 (hK2) is a prostate specific enzyme whose expression is governed by the androgen receptor (AR). AR is the central oncogenic driver of prostate cancer (PCa) and is also a key regulator of DNA repair in cancer. We report an innovative therapeutic strategy that exploits the hormone-DNA repair circuit to enable molecularlyspecific alpha particle irradiation of PCa. Alpha-particle irradiation of PCa is prompted by molecularly specific-targeting and internalization of the humanized monoclonal antibody hu11B6 targeting hK2 and further accelerated by inherent DNA-repair that up-regulate hK2 (KLK2) expression in vivo. hu11B6 demonstrates exquisite targeting specificity for KLK2. A single administration of actinium-225 labeled hu11B6 eradicates disease and significantly prolongs survival in animal models. DNA damage arising from alpha particle irradiation induces AR and subsequently KLK2, generating a unique feed-forward mechanism, which increases binding of hu11B6. Imaging data in nonhuman primates support the possibility of utilizing hu11B6 in man.

Calculating shock Hugoniot and isentropes using multiphase equation of state tables and application to shock and release of diamond ablators in inertial confinement fusion implosions

Advances in shock and ramp compression techniques now allow experimental access to unprecedented extreme conditions of pressure and temperature, providing a means to test theoretical models. Here, we describe a simple methodology to compute multi-phase shock Hugoniot and isentropes using multiphase equation of state tables. We treat explicitly the phase coexistence along the phase boundary to reveal the evolution of the sample as it undergoes the phase transformation in adiabatic conditions. We illustrate the method by calculating the predicted shock and shock-and-release behavior of diamond at conditions relevant for the initial stage of inertial confinement fusion implosions.

Humidity determines snowpack ablation under a warming climate

Significance Changes in the amount and timing of snowmelt have large effects on water for society and ecosystems. Using long-term records from across the western United States, we demonstrate that atmospheric humidity is a major control on how seasonal snow responds to warming temperatures. Specifically, we observe an increase in the frequency and magnitude of episodic winter melt events under higher humidity that may alter the timing of water availability. In lower-humidity regions, however, warming is associated with increased sublimation and/or evaporation from the snowpack further reducing the amount of available water in these dry regions. Management approaches to address these changes in snowmelt water resources from continued warming will require improved estimation of variable and changing atmospheric humidity.

Systematic Source Determination and X-Ray Radiography Detection of Nonuniformities on High-Density Carbon Ablators

Recent deuterium-tritium (D-T)–layered implosion experiments at the National Ignition Facility have achieved a burning plasma and >1-MJ neutron yield. A series of repeat experiments have shown that the degree of performance is very likely dependent on capsule quality, including the quantity of what are collectively termed “high- Z particles.” These particles are detected on a custom-built radiography system, known as the Sagometer, during the final target qualification process. The term particles is misleading, as the source of these nonuniformities in the capsule images is uncertain; the term detection will be used instead. An increased number of D-T targets have been rejected at the final stages of production due to Sagometer detections. Late detections are deleterious in terms of loss of production parts, effort, and overall operating efficiency. In response, we undertook an effort to determine the origin of these detections and to ultimately mitigate target losses caused by them. Through careful testing and analysis, we have determined neither insufficient production cleanliness nor hohlraum shedding is responsible for the detections on the capsule. Here, we determined that the detections are inherent to the capsule and have made efforts to use the Zeiss Xradia to identify them earlier in the production process. While testing revealed the Xradia is not currently sufficient for identifying such particles using radiography images, we continue to look to other forms of metrology to down select the capsules early in the process.
