This document contains the Appendices to the report documenting the activities, findings, and NASA Engineering and Safety Center (NESC) recommendations of a multidiscipline team to independently assess the Constellation Program (CxP) Crew Exploration Vehicle (CEV) Parachute Assembly System (CPAS). The assessment occurred during a period of 15 noncontiguous months between December 2008 and April 2010, prior to the CPAS Project's Preliminary Design Review (PDR) in August 2010.

MRI-based Quantification of Optic Nerve Tortuosity and Subarachnoid Space 3D Geometry: Reliability Assessment

In some astronauts, long-duration space flight results in ophthalmic structure changes such as optic nerve (ON) kinking, ON distention, and globe flattening. Assessment of the ON and ON sheath (ONS) may provide insight into the mechanisms responsible for ophthalmic structure changes seen in a subset of astronauts. Automated and manual methods were developed to quantify 3D ON/ONS geometry and ON tortuosity.

Reliability and maintainability assessment factors for reliable fault-tolerant systems

A long term goal of the NASA Langley Research Center is the development of a reliability assessment methodology of sufficient power to enable the credible comparison of the stochastic attributes of one ultrareliable system design against others. This methodology, developed over a 10 year period, is a combined analytic and simulative technique. An analytic component is the Computer Aided Reliability Estimation capability, third generation, or simply CARE III. A simulative component is the Gate Logic Software Simulator capability, or GLOSS. The numerous factors that potentially have a degrading effect on system reliability and the ways in which these factors that are peculiar to highly reliable fault tolerant systems are accounted for in credible reliability assessments. Also presented are the modeling difficulties that result from their inclusion and the ways in which CARE III and GLOSS mitigate the intractability of the heretofore unworkable mathematics.

Assessing and Enabling Trustworthy Predictions for High-Consequence Decisions

Predictions from physics-based computational models provide critical information to inform high consequence decisions, e.g., engineering design decisions. The ability to assess the reliability of such predictions is therefore critical. However, to date, reliability assessment rely heavily on expert judgment and qualitative arguments. This report details the efforts of LDRD 233072 to develop quantitative methods to assess reliability of model predictions, especially in the context of simplifying assumptions that can impact their reliability.

Integrating Formal Methods and Testing 2002

Traditionally, qualitative program verification methodologies and program testing are studied in separate research communities. None of them alone is powerful and practical enough to provide sufficient confidence in ultra-high reliability assessment when used exclusively. Significant advances can be made by accounting not only tho formal verification and program testing. but also the impact of many other standard V&V techniques, in a unified software reliability assessment framework. The first year of this research resulted in the statistical framework that, given the assumptions on the success of the qualitative V&V and QA procedures, significantly reduces the amount of testing needed to confidently assess reliability at so-called high and ultra-high levels (10-4 or higher). The coming years shall address the methodologies to realistically estimate the impacts of various V&V techniques to system reliability and include the impact of operational risk to reliability assessment. Combine formal correctness verification, process and product metrics, and other standard qualitative software assurance methods with statistical testing with the aim of gaining higher confidence in software reliability assessment for high-assurance applications. B) Quantify the impact of these methods on software reliability. C) Demonstrate that accounting for the effectiveness of these methods reduces the number of tests needed to attain certain confidence level. D) Quantify and justify the reliability estimate for systems developed using various methods.

DuraMAT Technology Scouting Report: Assessing Module Reliability Risks Associated with Projected Technological Changes

Maintaining the reliability of photovoltaic (PV) modules in the face of rapidly changing technology is critical to maximizing solar energy's contribution to global decarbonization. Our presentation describes expected changes in PV technology and their impacts on performance and reliability. We leverage PV market reports, interviews with PV researchers and other industry stakeholders, and peer-reviewed literature to narrow the multitude of possible changes into a manageable set of 11 impactful trends likely to be incorporated in near-term crystalline-silicon module designs. We group the trends into four categories (module architecture, interconnect technologies, bifacial modules, and cell technology) and explore the drivers behind the changes, their interactions, and associated reliability risks and benefits. Our analysis identifies specific areas that would benefit from accelerating the PV reliability learning cycle, to assess emerging module products and designs more accurately. We recommend that researchers continue tracking module technologies and their reliability implications so efforts can be focused on the most impactful trends. As the rapid technological turnover continues, it is also critical to incorporate fundamental knowledge into models that can predict module reliability. Predictive capabilities complete the PV reliability learning cycle-reducing the time required to assess new designs and mitigating the risks associated with large-scale deployment of new products.

Risk-informed Graded Approach for Reliability and Performance Assessment of Machine Learning and Artificial Intelligence for Advanced Condition Monitoring Techniques

With the shift away from time-based maintenance and toward condition-based maintenance, and to reduce overall maintenance costs, there has been an upsurge in the usage and development of advanced condition monitoring (ACM) techniques for real-time monitoring of nuclear power plant (NPP) components. ACM is particularly useful in the development of digital twins, which are designed to predict the failure or degradation of plant components. Successful implementation of ACM requires an assessment to inform the development of a risk-informed approach to evaluate the use of ACM to meet Nuclear Regulatory Committee (NRC) regulations for in-service testing (IST) programs. This includes the monitoring and diagnostics of reactor components and systems in current, new, and advanced reactors. A key component in ACM is the usage of machine learning (ML) and artificial intelligence (AI) algorithms that can employ real-time data from instrumentation and sensors to detect and predict reactor component degradations. Such predictive capabilities enable early detection of component degradation so as to help plant personnel plan and execute necessary maintenance. For successful implementation of ML/AI in ACM such that regulatory requirements are met, a risk-informed graded approach is needed to assess the reliability and performance of ML/AI for ACM. The American Society for Mechanical Engineers (ASME) developed their Operations and Maintenance (O&M) Code to provide guidance on safe, reliable O&M of NPPs. The IST section of the O&M Code specifically establishes requirements for IST and examination to gauge operational readiness of components in water-cooled NPPs. This paper presents a state-of-the-art review of how reliability and risk assessment can be integrated with ACM to assess component performance by non-nuclear industries. This is followed by different methodologies and approaches for conducting performance and reliability assessments so as to meet IST requirements for NPP components.
