Peripheral venous scintillation angiocardiography in determination of left ventricular volume in man.

Left ventricular end-diastolic volume was determined by cardiac visualization after peripheral venous injection of a gamma-emitting isotope in 10 patients with organic heart disease. The left ventricular end-diastolic volume measured by the isotope method consistently averaged 9% less than that determined by the X-ray method. The mean difference in left ventricular end-diastolic volume was 21 ml. Excellent correlation between the two methods was observed. It is pointed out that peripheral venous scintillation angiocardiography compares well with left ventriculography in the determination of left ventricular end-diastolic volume in man.

Parathyroid hormone - Secretion and metabolism in vivo.

Gel filtration and radioimmunoassay were used to determine the molecular size and immunochemical reactivity of parathyroid hormone present in gland extracts, in the general peripheral circulation, and in parathyroid effluent blood from patients with hyperparathyroidism, as well as from calves and from cattle. It was found that parathyroid hormone secreted from the parathyroids in man and cattle is at least as large as the molecule extracted from normal bovine glands. However, once secreted into the circulation the hormone is cleaved, and one or more fragments, immunologically, dissimilar to the originally secreted hormone, constitute the dominant form of circulating immunoreactive hormone.

Acquisition of electroencephalographic data in a large regional hospital - Bringing the brain waves to the computer.

This paper describes a complete electroencephalographic acquisition and transmission system, designed to meet the needs of a large hospital with multiple critical care patient monitoring units. The system provides rapid and prolonged access to a centralized recording and computing area from remote locations within the hospital complex, and from locations in other hospitals and other cities. The system includes quick-on electrode caps, amplifier units and cable transmission for access from within the hospital, and EEG digitization and telephone transmission for access from other hospitals or cities.

Densitography - A new method for evaluation of cardiac performance of rest and during exercise.

The validity of the densitographic curves obtained from the pinna of the ear for measuring the systolic intervals has been investigated in 28 subjects. This technique is found to permit the performance of noninvasive physiologic studies during physical activities, exercise tests, flight conditions, and probably long-term monitoring of critically ill patients. Densitographic curves constitute an excellent substitute by which to measure time-base intervals of the cardiac cycle when the carotid pulse tracing is not available.

Biotelemetry and computer analysis of sleep processes on earth and in space.

Developments in biomedical engineering now permit study of states of sleep, wakefulness, and focused attention in man exposed to rigorous environments, including aerospace flight. These new sensing devices, data acquisition systems, and computational methods have also been extensively applied to clinical problems of disordered sleep. A 'library' of EEG data has been prepared for sleep in normal man, and characterized for its group features by computational analysis. Sleep in an astronaut in space flight has been examined for the first and second 'nights' of space flight. Normal 90-min cycles were detected during the second night. Sleep patterns in quadriplegic patients deprived of all sensory inputs below the neck have indicated major deviations.

Past and future space flight experiments on man and their spin-off contribution to world health.

The manned space-flight program has created some requirements which were new to the practice of medicine. The remoteness of the patient made it necessary to provide sensing and transmitting capability for various physiologic functions. The space and weight penalties in a spacecraft required that such sensors and related equipment be miniaturized and of a noninterfering nature as far as the crewmen were concerned. The need for continually increasing flight durations created a requirement for a type of equipment reliability previously unknown to medicine. The requirement to protect the crewmen against the environment of space and provide a habitable spacecraft has produced a series of ground-based medical applications.

Advancements in medicine from aerospace research

A program designed to find second applications for space technology in the medical field is described. Illustrative examples and clinical test results are included for prosthetic urethral devices, ear oximeter for monitoring leukemia patients, devices for measuring low level CO effects on automobile drivers, radiation dosimeter probe for detecting radiation levels in cancerous areas, and electromyographic muscle trainer.

Noninvasive measurement of central venous pressure

A technique for the noninvasive measurement of CVP in man was developed. The method involves monitoring venous velocity at a point in the periphery with a transcutaneous Doppler ultrasonic velocity meter while the patient performs a forced expiratory maneuver. The idea is the CVP is related to the value of pressure measured at the mouth which just stops the flow in the vein. Two improvements were made over the original procedure. First, the site of venous velocity measurement was shifted from a vein at the antecubital fossa (elbow) to the right external jugular vein in the neck. This allows for sensing more readily events occurring in the central veins. Secondly, and perhaps most significantly, a procedure for obtaining a curve of relative mean venous velocity vs mouth pressure was developed.

Programmed Multiphasic Health Testing

Multiphase health screening procedures are advocated for detection and prevention of disease at an early stage through risk factor analysis. The use of an automated medical history questionnaire together with scheduled physical examination data provides a scanning input for computer printout. This system makes it possible to process laboratory results from 1,000 to 2,000 patients for biochemical determinations on an economically feasible base.

Coronary Risk Factor Scoring as a Guide for Counseling

A risk factor scoring system for early detection, possible prediction, and counseling to coronary heart disease patients is discussed. Scoring data include dynamic EKG, cholesterol levels, triglycerine content, total lipid level, total phospolipid levels, and electrophoretic patterns. Results indicate such a system is effective in identifying high risk subjects, but that the ability to predict exceeds the ability to prevent heart disease or its complications.

An Operating Environmental Health Program

Some concepts of an operational program for medical and environmental health are outlined. Medical services of this program are primarily concerned with emergency care, laboratory examinations, advice to private physician with patient permission, medical monitoring activities, and suggestions for treatment or control of the malfunction.

Specialized physiological studies in support of manned space flight

The areas of physiological research reported include: (1) evaluation of the single-breath method for determining cardiac output, (2) optimum protocol for the assessment of cardio-pulmonary competence, (3) body fluids and electrolytes under conditions of single and combined stress, (4) re-evaluation of the open-circuit method for measuring metabolic rate with regard to the alleged metabolic production of gaseous nitrogen, and (5) the use of the forced-oscillation method to determine total respiratory conductance in healthy subjects and pulmonary patients.

Operating Room Environment Control. Part A: a Valve Cannister System for Anesthetic Gas Adsorption. Part B: a State-of-the-art Survey of Laminar Flow Operating Rooms. Part C: Three Laminar Flow Experiments

An anesthetic gas flow pop-off valve canister is described that is airtight and permits the patient to breath freely. Once its release mechanism is activated, the exhaust gases are collected at a hose adapter and passed through activated coal for adsorption. A survey of laminar air flow clean rooms is presented and the installation of laminar cross flow air systems in operating rooms is recommended. Laminar flow ventilation experiments determine drying period evaporation rates for chicken intestines, sponges, and sections of pig stomach.

The use of a tracking test battery in the quantitative evaluation of neurological function

A number of tracking tasks that have proven useful to control engineers and psychologists measuring skilled performance have been evaluated for clinical use. Normal subjects as well as patients with previous diagnoses of Parkinson's disease, multiple sclerosis, and cerebral palsy were used in the evaluation. The tests that were studied included step tracking, random tracking, and critical tracking. The results of the present experiments encourage the continued use of tracking tasks as assessment precedures in a clinical environment. They have proven to be reliable, valid, and sensitive measures of neurological function.
