Assessment of the NASA Space Shuttle Program's Problem Reporting and Corrective Action System

This paper documents the general findings and recommendations of the Design for Safety Programs Study of the Space Shuttle Programs (SSP) Problem Reporting and Corrective Action (PRACA) System. The goals of this Study were: to evaluate and quantify the technical aspects of the SSP's PRACA systems, and to recommend enhancements addressing specific deficiencies in preparation for future system upgrades. The Study determined that the extant SSP PRACA systems accomplished a project level support capability through the use of a large pool of domain experts and a variety of distributed formal and informal database systems. This operational model is vulnerable to staff turnover and loss of the vast corporate knowledge that is not currently being captured by the PRACA system. A need for a Program-level PRACA system providing improved insight, unification, knowledge capture, and collaborative tools was defined in this study.

Lessons Learned for Cx PRACA. Constellation Program Problem Reporting, Analysis and Corrective Action Process and System

This slide presentation reviews the Constellation Program Problem Reporting, Analysis and Corrective Action Process and System (Cx PRACA). The goal of the Cx PRACA is to incorporate Lessons learned from the Shuttle, ISS, and Orbiter programs by creating a single tool for managing the PRACA process, that clearly defines the scope of PRACA applicability and what must be reported, and defines the ownership and responsibility for managing the PRACA process including disposition approval authority. CxP PRACA is a process, supported by a single information gathering data module which will be integrated with a single CxP Information System, providing interoperability, import and export capability making the CxP PRACA a more effective and user friendly technical and management tool.

ATM C and D panel/EREP cooling system contamination problem

This report presents the history of a preflight contamination problem that occurred in the ATM C and D panel/EREP cooling system on the Skylab, the studies that were made to determine the cause of the problem, and corrective actions that were made prior to lift-off. The results of all the observations, analyses and laboratory testing indicated that the contamination came from one or more of the EREP tape recorder coldplates and was caused by some abnormal electrolytic action, either during bench testing or in the spacecraft. Studies indicate that no such electrolytic action is likely to occur under normal operating conditions.

Automatic vehicle monitoring systems study. Report of phase O. Volume 2: Problem definition and derivation of AVM system selection techniques

A set of planning guidelines is presented to help law enforcement agencies and vehicle fleet operators decide which automatic vehicle monitoring (AVM) system could best meet their performance requirements. Improvements in emergency response times and resultant cost benefits obtainable with various operational and planned AVM systems may be synthesized and simulated by means of special computer programs for model city parameters applicable to small, medium and large urban areas. Design characteristics of various AVM systems and the implementation requirements are illustrated and cost estimated for the vehicles, the fixed sites and the base equipments. Vehicle location accuracies for different RF links and polling intervals are analyzed. Actual applications and coverage data are tabulated for seven cities whose police departments actively cooperated in the study.

Discovering Recurring Anomalies in Text Reports Regarding Complex Space Systems

Many existing complex space systems have a significant amount of historical maintenance and problem data bases that are stored in unstructured text forms. For some platforms, these reports may be encoded as scanned images rather than even searchable text. The problem that we address in this paper is the discovery of recurring anomalies and relationships between different problem reports that may indicate larger systemic problems. We will illustrate our techniques on data from discrepancy reports regarding software anomalies in the Space Shuttle. These free text reports are written by a number of different penp!e, thus the emphasis and wording varies considerably.

Robot, computer problem solving system

The development of a computer problem solving system is reported that considers physical problems faced by an artificial robot moving around in a complex environment. Fundamental interaction constraints with a real environment are simulated for the robot by visual scan and creation of an internal environmental model. The programming system used in constructing the problem solving system for the simulated robot and its simulated world environment is outlined together with the task that the system is capable of performing. A very general framework for understanding the relationship between an observed behavior and an adequate description of that behavior is included.

Modeling to Improve the Risk Reduction Process for Command File Errors

The Jet Propulsion Laboratory has learned that even innocuous errors in the spacecraft command process can have significantly detrimental effects on a space mission. Consequently, such Command File Errors (CFE), regardless of their effect on the spacecraft, are treated as significant events for which a root cause is identified and corrected. A CFE during space mission operations is often the symptom of imbalance or inadequacy within the system that encompasses the hardware and software used for command generation as well as the human experts and processes involved in this endeavor. As we move into an era of increased collaboration with other NASA centers and commercial partners, these systems become more and more complex. Consequently, the ability to thoroughly model and analyze CFEs formally in order to reduce the risk they pose is increasingly important. In this paper, we summarize the results of applying modeling techniques previously developed to the DAWN flight project. The original models were built with the input of subject matter experts from several flight projects. We have now customized these models to address specific questions for the DAWN flight project and formulating use cases to address their unique mission needs. The goal of this effort is to enhance the project's ability to meet commanding reliability requirements for operations and to assist them in managing their Command File Errors.

Robot computer problem solving system

Continuing research is reported in a program aimed at the development of a robot computer problem solving system. The motivation and results are described of a theoretical investigation concerning the general properties of behavioral systems. Some of the important issues which a general theory of behavioral organization should encompass are outlined and discussed.

Multi-system report integration using blackboards

Blackboards are an Al problem solving methodology. A blackboard system consists of a structured data base (the blackboard) holding input and derived inferences and a collection of procedures for deriving inferences (knowledge sources). Each knowledge source is specialized to operate on some portion of the blackboard. The knowledge sources are invoked opportunistically as the information on the blackboard increases. The methodology is well suited to integrating multiple streams of fully reduced and qualitatively different data such as active radar track reports, passive electronic intelligence reports, and human intelligence reports about enemy intentions. This paper sketches the nature of the blackboard problem solving methodology with an emphasis on those features suiting it to such applications. The sketch is illustrated with examples from a relatively simple multi-system report integration problem. Relevant applications currently under development at Stanford's Knowledge Systems Laboratory are also described.

Flight set 360L007 (STS-33R) field joint protection system, thermal protection system, and systems tunnel components, volume 7

The performance of the thermal protection system, field joint protection system, and systems tunnel components of flight set 360L007 is presented as evaluated by postflight hardware inspection. The condition of both motors was similar to previous flights. Four aft edge strikes were noted on the ground environment instrumentation thermal protection system. The hits all left a clean substrate, indicating that the damage was caused by nozzle severance debris and/or water impact. No National Space Transportation System debris criteria for missing thermal protection system were violated. Two problem reports were written against the field joint protection system. The first concerned two cracks in the K5NA closeout over the trunnion/vent valve location on the left-hand aft field joint. A similar condition was observed on Flight 5 (360H005). The second problem report referred to a number of small surface cracks between two impact marks on the left-hand forward field joint. Neither area exhibited loose material or any abnormal heat effects, and they have no impact on flight safety.

Intelligent Text Retrieval and Knowledge Acquisition from Texts for NASA Applications: Preprocessing Issues

A system that retrieves problem reports from a NASA database is described. The database is queried with natural language questions. Part-of-speech tags are first assigned to each word in the question using a rule based tagger. A partial parse of the question is then produced with independent sets of deterministic finite state a utomata. Using partial parse information, a look up strategy searches the database for problem reports relevant to the question. A bigram stemmer and irregular verb conjugates have been incorporated into the system to improve accuracy. The system is evaluated by a set of fifty five questions posed by NASA engineers. A discussion of future research is also presented.

Reliability inputs to Mariner 9 data explosion

This paper describes the prelaunch and post-launch reliability functions which contributed to the success of the Mariner 9 spacecraft. Examples are included to illustrate how each reliability activity was a vital part of each phase of the project. Prelaunch reliability functions included: (1) establishing, negotiating, and monitoring system and subsystem requirements, (2) participating in spacecraft system and subsystem design/hardware reviews, (3) monitoring preparation of failure mode effects and criticality analyses (FMECA), (4) establishing and managing a problem failure reporting (PFR) system for the spacecraft and its support equipment, (5) monitoring electronic parts activities, and (6) participating in spacecraft reviews. Particular emphasis is placed on mission operations reliability assurance activities, which included: (1) spacecraft problem/failure reporting, (2) managing an integrating failure reporting system which covered all mission operations activities, (3) real-time analysis of spacecraft anomalies, and (4) risk assessment.

NASA/DOD Aerospace Knowledge Diffusion Research Project. Report 12: An initial investigation into the production and use of Scientific and Technical Information (STI) at five NASA centers: Results of a telephone survey

A study was conducted to provide NASA management with an 'initial' look at the production and use of scientific and technical information (STI) at five NASA centers (Ames, Goddard, Langley, Lewis, and Marshall). The 550 respondents who were interviewed by telephone held favorable views regarding the NASA STI system. About 65 percent of the respondents stated that it is either very or somewhat important for them to publish their work through the NASA STI system. About 10 percent of those respondents encountered problems using the NASA STI system services for publication. The most frequently reported problem was 'the process is too time consuming' (8.6 percent). Overall, those respondents using the NASA STI system to publish their work rated the system as excellent (24.6 percent) or good (37.6 percent). About 79 percent of the respondents stated that it is either very or somewhat important for them to use the NASA STI system to access information. The most frequently reported problems were 'the time and effort it takes to locate and obtain information through the system' (14.4 percent). Overall, about 83 percent of the respondents stated that the NASA STI system is important to performing their work. Overall, about 73 percent of the respondents stated that the NASA STI system meets their information needs.

Potential effects of the introduction of the discrete address beacon system data link on air/ground information transfer problems

This study of Aviation Safety Reporting System reports suggests that benefits should accure from implementation of discrete address beacon system data link. The phase enhanced terminal information system service is expected to provide better terminal information than present systems by improving currency and accuracy. In the exchange of air traffic control messages, discrete address insures that only the intended recipient receives and acts on a specific message. Visual displays and printer copy of messages should mitigate many of the reported problems associated with voice communications. The problems that remain unaffected include error in addressing the intended recipient and messages whose content is wrong but are otherwise correct as to format and reasonableness.
