To Create Safety Is Human

Abstract

It is often said that to err is human. It's true that failures can be traced to human limitation, but what's more important is that all successes, all safe operations, are the result of human capabilities. This talk highlights the resilience people bring to aviation operations and discusses ways to change the common narrative that people are the creators of safety rather than only the source of error and failure.

Operations: A Wholistic View

An operation is often constructed of discrete parts. These parts are often designed, developed, and practiced independently of each other. However, to work together effectively and efficiently, these parts must constitute a unified whole. To achieve its operational goals, this wholeness must be guided by a clear, coherent, consistent and comprehensive framework of procedures and policies. In this talk, I describe such a framework, motivate its structure, and demonstrate its effectiveness.

Lunar Prospecting: Searching for Volatiles at the South Pole

The Resource Prospector is an in-situ resource utilization (ISRU) technology demonstration mission, planned for a 2021 launch to search for and analyze volatiles at the Lunar South Pole. The mission poses unique operational challenges. Operating at the Lunar South Pole requires navigating a surface with lighting, shadow and regolith characteristics unlike those of previous missions. The short round trip communications time enables reactive surface operations for science and engineering. Navigation of permanently shadowed regions with a solar powered rover creates risks, including power and thermal management, and requires constant real time decision making for safe entry, path selection and egress. The mission plan requires a faster rover egress from the lander than any previous NASA rover mission.

Evolution and Implementation of the NASA Robotic Conjunction Assessment Risk Analysis Concept of Operations

Reacting to potential on-orbit collision risk in an operational environment requires timely and accurate communication and exchange of data, information, and analysis to ensure informed decision-making for safety of flight and responsible use of the shared space environment. To accomplish this mission, it is imperative that all stakeholders effectively manage resources: devoting necessary and potentially intensive resource commitment to responding to high-risk conjunction events and preventing unnecessary expenditure of resources on events of low collision risk. After 10 years of operational experience, the NASA Robotic Conjunction Assessment Risk Analysis (CARA) is modifying its Concept of Operations (CONOPS) to ensure this alignment of collision risk and resource management. This evolution manifests itself in the approach to characterizing, reporting, and refining of collision risk. Implementation of this updated CONOPS is expected to have a demonstrated improvement on the efficacy of JSpOC, CARA, and owner/operator resources.
