Operating Experience Data Analysis for Digital Instrumentation and Control System Reliability and Risk Assessment in Nuclear Power Plants

The implementation of advanced digital instrumentation and control (DI&C) systems in U.S. nuclear power plants (NPPs) can bring significant advancements in reliability, monitoring, and control capabilities. However, these systems also introduce new challenges, particularly in assessing risks such as common-cause failures (CCFs) and establishing robust reliability estimates for DI&C components. Addressing these challenges is critical for ensuring the safe and efficient operation of NPPs. Recently, Idaho National Laboratory was tasked by the U.S. Nuclear Regulatory Commission (NRC) to conduct a DI&C reliability study using operating experience data from the nuclear industry. The two operating experience data sources for the study are the Institute of Nuclear Power Operations’ Industry Reporting and Information System (IRIS) and the NRC’s Licensee Event Report database which is hosted at Idaho National Laboratory at https://lersearch.inl.gov/LERSearchCriteria.aspx. This report provides a comprehensive examination of DI&C systems, including their architecture, operational advantages, and associated challenges. It reviews existing industry DI&C studies and failure mode taxonomies, along with reliability data from various industries. Through a detailed analysis of these databases, the study provides insights into DI&C system performance. Considerations should be given to incorporate DI&C failure data into the NRC's Integrated Data Collection and Coding System and updating the Reliability and Availability Data System to support ongoing DI&C reliability studies. Recommendations are also provided for modeling DI&C reliability and CCF in probabilistic risk assessment, thereby supporting risk-informed decision-making and enhancing the reliability and safety of NPPs.

System Study: Reactor Core Isolation Cooling 1998-2024

This report presents an unreliability evaluation of the reactor core isolation cooling (RCIC) system at 28 U.S. commercial operating boiling water reactors. New Standardized Plant Analysis Risk (SPAR) models with the most recent SPAR parameter update results were used in this report. Demand, run hour, and failure data from calendar years 1998 to 2024 for selected components were obtained from the Institute of Nuclear Power Operations Industry Reporting and Information System. The unreliability results are trended for the most recent 10 year period and yearly estimates for system unreliability are provided for the entire active period. No statistically significant increasing or decreasing trends were identified in the RCIC results.

Enhanced Component Performance Study: Motor Driven Pumps 1998-2024

This report presents an enhanced performance evaluation of motor-driven pumps (MDPs) at U.S. commercial nuclear power plants. The data used in this study are based on the operating experience failure reports from calendar year 1998 through 2024 as reported in the Institute of Nuclear Power Operations (INPO) Industry Reporting and Information System (IRIS). The MDP failure modes considered for standby systems are fail to start (FTS), fail to run (FTR) for one hour of operation (FTR=1H), FTR after one hour of operation (FTR>1H), and for normally running systems FTS and FTR. An eight-hour unreliability estimate is also calculated and trended. The component reliability estimates and the reliability data are trended for the most recent 10-year period while yearly estimates for reliability are provided for the entire study period.

The component reliability estimates and the reliability data are trended for the most recent 10 year period while yearly estimates for reliability are provided for the entire study period. No increasing trends were identified for TDPs for the most recent 10 year period.

System Study: Auxiliary Feedwater 1998-2024

This report presents an unreliability evaluation of the auxiliary feedwater (AFW) system at 62 U.S. commercial operating nuclear reactors. New Standardized Plant Analysis Risk (SPAR) models with the most recent SPAR parameter update results were used in this report. Demand, run hour, and failure data from calendar years 1998 to 2024 for selected components were obtained from the Institute of Nuclear Power Operations Industry Reporting and Information System. The unreliability results are trended for the most recent 10 year period and yearly estimates for system unreliability are provided for the entire active period. No statistically significant increasing or decreasing trends were identified in the industry-wide estimates of AFW system start-only unreliability, but a statistically significant decreasing trend was identified in the industry-wide estimates of AFW system 24-hour mission unreliability.

System Study: Residual Heat Removal 1998-2024

This report presents an unreliability evaluation of the residual heat removal (RHR) system in two modes of operation (low-pressure injection in response to a large loss-of-coolant accident and in response to post-trip shutdown cooling) at 93 U.S. commercial operating nuclear reactors. New Standardized Plant Analysis Risk (SPAR) models with the most recent SPAR parameter update results were used in this report. Demand, run hour, and failure data from 1998 to 2024 for selected components were obtained from the Institute of Nuclear Power Operations (INPO) Industry Reporting and Information System (IRIS), formerly the INPO Consolidated Events Database. The unreliability results are trended for the most recent 10 year period and yearly estimates for system unreliability are provided for the entire active period. Statistically significant and highly statistically significant decreasing trends were observed for RHR low-pressure injection mode start-only unreliability and RHR low-pressure injection model 24-hour unreliability.

A knowledge-informed large language model framework for U.S. nuclear power plant shutdown initiating event classification for probabilistic risk assessment

Identifying and classifying shutdown initiating events (SDIEs) is critical for developing shutdown probabilistic risk assessment for nuclear power plants. Existing computational approaches cannot achieve satisfactory performance due to the challenges of unavailable large, labeled datasets, imbalanced event types, and label noise. To address these challenges, we propose a hybrid pipeline that integrates a knowledge-informed machine learning model to prescreen non-SDIEs and a large language model (LLM) to classify SDIEs into four types. In the prescreening stage, we proposed a set of 44 SDIE text patterns that consist of the most salient keywords and phrases from six SDIE types. Text vectorization based on the SDIE patterns generates feature vectors that are highly separable by using a simple binary classifier. The second stage builds Bidirectional Encoder Representations from Transformers (BERT)-based LLM, which learns generic English language representations from self-supervised pretraining on a large dataset and adapts to SDIE classification by fine-tuning it on an SDIE dataset. The proposed approaches are evaluated on a dataset with 10,928 events using precision, recall ratio, F 1 score, and average accuracy. In conclusion, the results demonstrate that the prescreening stage can exclude more than 97% non-SDIEs, and the LLM achieves an average accuracy of 95.1% for SDIE classification.

The MOV failure modes considered are fail to open or close (FTOC), fail to operate or control (FTOP), and spurious operation (SO). The component reliability estimates and the reliability data are trended for the most recent 10-year period while yearly estimates for reliability are provided for the entire study period. The following increasing trend was identified for MOVs for the most recent 10-year period: • Low-demand MOV frequency of FTOC demands (demands per reactor year). The following decreasing trends were identified for MOVs for the most recent 10-year period: • Low-demand MOV FTOC failure probability • High-demand MOV SO failure rate • Low-demand MOV frequency of FTOC events (failures per reactor year) • High-demand MOV frequency of SO events (failures per reactor year).

Enhanced Component Performance Study: Emergency Diesel Generators 1998-2024

This report presents an enhanced performance evaluation of the emergency power system (EPS) and high-pressure core spray (HPCS) emergency diesel generators (EDGs) at U.S. commercial nuclear power plants. This report evaluates component performance over time using (1) Institute of Nuclear Power Operations (INPO) Industry Reporting and Information System (IRIS) data from 1998 through 2024 and (2) maintenance unavailability performance data from Mitigating Systems Performance Index (MSPI) Basis Document data from 2002 through 2024. The objective is to show estimates of current failure probabilities and rates related to EDGs, trend these data on an annual basis, determine if the current data are consistent with the probability distributions currently recommended for use in Nuclear Regulatory Commission (NRC) probabilistic risk assessments, show how the reliability data differ for different EDG manufacturers and for EDGs with different ratings; and summarize the subcomponents, causes, detection methods, and recovery associated with each EDG failure mode. The EDG failure modes considered are fail to start (FTS), fail to load and run (FTLR), and fail to run after one hour of operation (FTR>1H). Engineering analyses were performed with respect to time-period and failure mode without regard to the actual number of EDGs at each plant. The factors analyzed include subcomponent, failure cause, detection method, recovery, manufacturer, and EDG rating. The following increasing trends were identified for EDGs for the most recent 10-year period: • EPS and HPCS EDG frequency of start demands (demands per reactor year) • EPS and HPCS EDG frequency of FTLR demands • EPS and HPCS EDG frequency of run>1H hours. The following decreasing trends were identified for EDGs for the most recent 10-year period: • EPS EDG FTR>1H failure rate • EPS EDG unreliability • EPS and HPCS EDG frequency of FTR>1H events (failures per reactor year).
